Case Management
Guidance and Support for Every Step Toward Stability
Case Management provides individualized support to people experiencing homelessness or housing instability. Case managers work closely with residents across our various campus programs, including Our Place, Nevada Cares Campus, and Safe Camp, to assess needs, develop personalized plans, and connect individuals and families to the resources required for stability and long-term success.
Case management is designed for anyone navigating homelessness, whether residents are in temporary shelter, transitional programs, or long-term housing.
How We Support Program Residents
Individualized Planning: Case managers meet one-on-one with residents to understand their goals, challenges, and unique circumstances. Together, they create a roadmap for housing stability, health, and self-sufficiency.
Resource Coordination: Case management links residents to services both on and off-campus, including housing programs, healthcare, employment resources, benefits assistance, and educational opportunities.
Support Across Programs: Residents in shelters, Safe Camp, or Winter Overflow have access to case management services to ensure continuity of care. Case managers help coordinate services across programs so residents can seamlessly transition to the next step toward stability.
Crisis Support and Advocacy: When urgent needs arise, case managers assist with immediate concerns such as safety, food, medical care, and behavioral health. They advocate for residents with partner agencies and community programs to ensure access to resources.
Helping Residents Move Toward Stability
Case management is an important part of our shelter programs. By offering personalized guidance, connecting residents to services, and helping them navigate the complexities of homelessness, case managers empower individuals and families to move from crisis to stability. Through consistent support and advocacy, residents can regain independence, maintain housing, and build a foundation for long-term success.
